TST Forms

A number of forms have been developed to collect needed information and to enhance the efficient running of our programs and office. Some of these forms are filled out online and submitted directly from your computer to the office, staff and committees. Other forms, for security and privacy, are intended to be printed, completed and delivered to the office. 

Publicity Form (formerly Communication Form)

This is the form  (http://www.shirtikva.org/publicity-form) to publicize an event on the website calendar,Vats Nu, Hineni, Shir Tikva’s annual Program Book and/or town newspapers or on-line publications. It is filled out online and when you press “Submit” a copy goes to the office and to Shir Tikva editors. You must be logged in as a member to use the form. Here is a sample of a good event description:

Sisterhood Cooking Contest

The Sisterhood will hold a cooking contest on Monday, April 1, 2015. The Temple Shir Tikva kitchen will be turned upside down with creative interpretations of traditional Jewish foods including Shirley Tiket’s famous hot sauce for gefilte fish. If you have never been to a Sisterhood event don't miss this one. The event is free for all Shir Tikva members.  Pictures of last year’s winners are posted in the Photo Gallery. Abatement Form

To promote Shir Tikva’s commitment to welcoming all families who wish to be part of our community, the congregation has established a responsible policy regarding dues and fees abatement procedures. Abatements are considered after the filing of an Abatement Request Form and Application. That form, and a full explanation of the temple’s abatement policy, are at the link below.
